I emailed the KeePass author about these icons. He changed them in a
later version. Some are from openclipart.org, some are created by him
but he doesn't know where the mobile phone icons is from. I'll use
another mobile phone icon from openclipart.org.
They are all in the public domain.

Yeah, I used the ones from Oxygen.

It turns out they are from the Oxygen icon set (layer-visible-{on,off}.png).
